Pregnancy Outcome and Deliveries following Laparoscopic Myomectomy in Evercare Hospital Dhaka, Bangladesh

Abstract

Background: Uterine myomas, or fibroids, are common benign tumors that can impair fertility. Laparoscopic myomectomy (Laparoscopic Myomectomy) is a preferred surgery, reducing recovery time, minimizing adhesions, and preserving fertility. This study assesses pregnancy outcomes and delivery characteristics after Laparoscopic Myomectomy at Evercare Hospital, Dhaka, Bangladesh. Methods: This observational cross-sectional study was conducted from January 2021 to December 2024 in Evercare Hospital, Dhaka, Bangladesh, after fulfilling the exclusion and inclusion criteria by purposive sampling method. A total of 130 apparent Participants were included in the study, 73 patients who became pregnant following Laparoscopic Myomectomy were followed up for pregnancy-related outcomes, including mode of delivery, complications, and perinatal outcomes. Data on demographic characteristics, clinical factors, myoma details, surgical procedures, and pregnancy outcomes were collected. Results: The mean age of patients was 33.2 years, with the majority being nulliparous (79%). The most common type of myoma was subserous (41.8%), followed by intramural (32.6%). Pregnancy outcomes revealed that 69.0% of pregnancies resulted in successful deliveries, while 26.2% ended in spontaneous abortion. The mode of delivery was diverse, with 56.0% of patients having spontaneous vaginal delivery and 44.0% undergoing Caesarean section before labor. Obstetric complications included uterine rupture (3.0%) and post-partum hemorrhage (3.0%). Conclusion: Laparoscopic myomectomy demonstrates favorable pregnancy outcomes and is an effective option for women seeking fertility preservation while managing uterine myomas. However, the risk of uterine rupture and other complications exists, indicating the need for further research with larger cohorts and long-term follow-up to better understand the full scope of risks and benefits.
