Parasacral Ischial Plane Block with Femoral Block in High-Risk Patients of Below Knee Surgery: A Prospective Observational Study

Abstract
Background: High-risk patients undergoing below-knee surgeries require anesthetic techniques that ensure hemodynamic stability and effective analgesia.
Objective: To evaluate the block performance characteristics, hemodynamic stability, and analgesic efficacy of ultrasound-guided parasacral ischial plane block combined with femoral nerve block in high-risk patients.
Material and Methods: A prospective observational study was conducted in 70 ASA III and IV patients undergoing below-knee surgeries, assessing hemodynamic parameters, sensory and motor block characteristics, and duration of analgesia.
Results: Hemodynamic parameters remained stable intraoperatively with no significant deviations from baseline. Sensory and motor block onset times were predictable, and prolonged durations of blockade and analgesia were achieved.
Conclusion: Ultrasound-guided parasacral ischial plane block with femoral nerve block is a safe and effective regional anesthesia technique for below-knee surgeries in high-risk patients, providing excellent hemodynamic stability and prolonged postoperative analgesia.
